Partial Hospitalization Program (PHP)

PHP is the highest level of outpatient care we offer, and for BPD, it is often where treatment needs to start. Weekly therapy can teach coping skills, but it rarely provides enough structure to interrupt the patterns that keep emotional dysregulation, relationship conflict, and impulsive behavior cycling. PHP runs 5 days per week, 5 to 6 hours daily, for 4 to 8 weeks. That concentrated exposure is what makes the difference: you are practicing new skills in real time, with clinical support available when things get hard, not three days later at your next appointment.

Intensive Outpatient Program (IOP)

IOP provides structured treatment while you maintain work, school, or family responsibilities. For BPD, this level of care works best as a step-down from PHP or for people whose emotional dysregulation is real but not severe enough to require full-day programming. Treatment runs 3 days per week, 3 to 4 hours per session, with morning or afternoon scheduling options. Nervous System Regulation

The emotional intensity that comes with BPD is not a personality flaw. It is a nervous system that has a very narrow window of tolerance and tips into fight, flight, or shutdown faster than most people can track. EMDR and Brainspotting allow that processing to happen without requiring clients to retell the story in detail. IFS helps identify the protective parts that developed in response to those experiences and works with them rather than against them.

How do I know if I need more than weekly therapy for BPD?
If you have been in weekly therapy for months or years and the same patterns keep showing up, that is not a failure on your part. BPD involves nervous system dysregulation that happens faster than a once-a-week session can address. When the emotional flooding, relationship ruptures, or impulsive behavior is happening daily, you need daily structure to match it. Intensive treatment gives your brain and body enough repetition to build new patterns, not just talk about them.
PHP is five days a week, five to six hours a day. It is built for people whose dysregulation is severe enough that weekly therapy is not creating lasting change. IOP is three days a week, three hours a day, and works well as a step-down from PHP or for people with moderate symptoms who can maintain some daily structure on their own. Both programs include DBT skills training, individual therapy, and somatic work. Your clinical team recommends which level fits based on your assessment.
Yes, and that is usually the part people with BPD are most nervous about. Group feels risky when your emotions are intense and your relationships are unpredictable. But group is where the real skill-building happens, because you are practicing distress tolerance and interpersonal effectiveness with other people in real time, not hypothetically. The groups are small, structured, and clinician-led. Most clients say group became the most useful part of their treatment.
BPD brains tend to show specific patterns on qEEG, particularly in areas that regulate emotional reactivity and impulse control. Neurofeedback trains those areas directly. It does not replace therapy, but it gives your nervous system a baseline of regulation that makes therapy more effective. Most clients start noticing reduced emotional intensity within the first two to three weeks.
That is the norm, not the exception. BPD frequently co-occurs with depression, anxiety, PTSD, substance use, and eating disorders.
